Sports Med Group 2022

The Sports Medicine team at Holy Innocents’ consists of three full-time, nationally-certified and state-licensed athletic trainers. This team works closely with Dr. Ashok Reddy and Dr. Robertson with Peachtree Orthopedics. As a team, we hope to provide compassionate care in the prevention, examination, diagnosis, treatment and rehabilitation of emergent, acute or chronic injuries and medical conditions.

Each season, we have a team of high school students that help in the athletic training room and on the sidelines gaining first-hand experience and knowledge about athletic injuries and the athletic training profession.

The Sports Medicine Office opens daily at 1 p.m. and we are at our busiest right after school. If we are unable to get to the phone right away or are out of the office, please leave us a message so we can get back to you ASAP.

MEDICAL Forms

In order for your child to be able to participate in tryouts and begin their season, the following items are required. All items must be completed and uploaded into the Magnus Health online portal before the first day of tryouts. Health forms can be accessed through your myHIES parent account under the Resources tab and then Magnus.

 

1 - Athletic Physical: All physicals must be completed on the GHSA form. These forms can be found in Magnus Health. Physicals cannot expire between the first date of tryouts and the date of the last regular season game of the season. If your child has a physical that expires during the season, a new physical that does not expire during the season will need to be uploaded before participation will be allowed. We do accept physicals from Minute Clinics and Urgent cares as long as they are completed on the GHSA form.

 

Any physical completed between 4/1/2023-7/31/2023 remains valid for the entire 2023-2024 school year!

2 - The following forms must be completed online and can be found on Magnus Health.  

Athletic Waiver-Signed once 

Concussion Awareness - signed annually

Sudden Cardiac Arrest - signed annually

3 - In addition, those in high school must complete a baseline Impact Test. Baseline impact tests are done every two calendar years. Do not complete a new one with each season.

At the end of the Impact test, please download the certificate as a pdf and upload to Magnus Health Portal 


***Reminder: When seeing a physician for any injury, illness, and/or skin disorder, we do require a note from the physician before your child is eligible to return to play. If we do not receive a note, they will not be permitted to participate until one is produced***

*If you feel your child may have sustained a possible concussion, please notify the Sports Medicine team ASAP for an evaluation. Please remove them from all physical and athletic activity until cleared by an appropriate health-care professional trained in the management of concussions.
